This is because for starters, the market isn't very large (small town) and even better, you have it locked down. For example, with our local dentist out in the sticks, if his goal is to be the biggest dentist in the county, unless he gets ripped off, his SEO costs won't be that much, comparatively.

As a general rule of thumb for doctors, dentists, etc. , you should be able to find good, quality SEO starting around the $1000/month to $1,500/month range, depending on your goals, location and market.
These prices go on up as big as your budget, and can vary greatly depending on your goals, vision, how much content marketing is included, etc.
